The train, which was part of the Kitchener line, was departing Union Station at a low speed when it derailed. Preliminary reports indicate that rail fastenings on the south track of depot rail three failed. This was caused by screws that “sheared” under duress and eventually gave way.

After the train derailment, it pulled through two switches, damaging critical infrastructure, including signaling systems. CEO Michael Lindsay highlighted that this incident resulted in operational difficulties for GO Transit.
