Denbigh Hall South Junction Derailment Report Released: April 2026

An out-of-service passenger train derailed at Denbigh Hall South Junction on 26 June 2025. The incident occurred about 12:27 on the West Coast Main Line, between Bletchley and Milton Keynes Central.

The train was moving at about 15 mph (24 km/h). It was travelling from Bletchley to a depot in Northampton.

What happened

The movement was a wrong-direction manoeuvre. A fault prevented the driver from operating from one cab, so the train needed to reverse direction.

The train ran over switch diamond points that were set incorrectly for its direction. The leading vehicle then derailed, causing damage to the train and to track infrastructure.

Signalling and operational actions

Signallers at Rugby Signalling Control Centre planned and authorised the path for the out-of-course movement. They believed the path was valid but it was not.

  • Network Rail should train signallers on tools and techniques to set up and check proposed paths during out-of-course events.
  • Network Rail and West Midlands Trains should develop training on switch diamond points, to ensure staff meet Rule Book requirements.
  • The Rail Safety and Standards Board should review whether the Rule Book should address a signal at danger near the start of a planned wrong-direction movement.
